Our employees are our greatest asset. As part of our compensation package, we’re committed to providing a comprehensive group benefits plan.
We’ve attached an information package that provides an overview of the coverage provided through our insurance carrier, Great-West Life. Refer to the information in this package for details on the products and services available under our plan.
You’ll receive a copy of your Group Benefit Plan employee booklet shortly. The booklet provides details including coverage amounts, deductibles and plan maximums by benefit type. Be sure to sign up to Great-West’s GroupNet for Plan Members at www.greatwestlife.com<http://www.greatwestlife.com> for access to online claims submission, direct deposit of claim payments, to view the status of your claims and more.
You’ll also receive Great-West benefits cards, one for you and one for your spouse, if applicable. These cards provide your group plan number and member ID. Keep the card with you at all times and use it when this information is requested by a provider, for instance, when having prescriptions filled, and when traveling in case of medical emergency.
Plan member responsibilities
If you experience a major lifestyles event, you must advise your plan administrator within 31 days of the event. If Great-West isn’t notified of a lifestyle event within 31 days of the event, medical evidence may be required, limitations may be placed on coverage, or coverage may be declined.
Major lifestyles events include:
• Marriage (legal or common-law)
• Legal Separation
• Divorce or termination of a common-law relationship
• Death of spouse or insured child
• Acquisition of a common-law or same-sex spouse
• Termination of a common-law or same-sex relationship
• Birth or adoption of a child
• Loss of spouse’s benefits
• Dependants becoming ineligible for dependant coverage either because of their age or student status.
